This Notice applies to the protected health information (PHI) we create or receive as part of your care at Abbott Health & Wellness LLC. It is separate from our website Privacy Policy, which covers information collected through this website only.

Our Commitment to Your Privacy

We are required by law to maintain the privacy of your protected health information, to provide you with this Notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI. We are required to abide by the terms of the Notice currently in effect.

How We May Use and Disclose Your Health Information

For Treatment

We may use and share your health information to provide, coordinate, or manage your care — for example, sharing information between your provider, our medical assistants, and our med spa team, or with a specialist we refer you to.

For Payment

We may use and share your health information to bill and receive payment for the services you receive, including verifying insurance coverage or processing card payments.

For Healthcare Operations

We may use and share your information for internal operations, such as quality improvement, staff training, and business planning.

Other Permitted or Required Uses

Appointment reminders and information about treatment alternatives or health-related benefits and services that may interest you
As required by law, including public health reporting, health oversight activities, and in response to a valid court order or subpoena
To avert a serious threat to health or safety
Workers' compensation, if applicable to your visit
Business associates who perform services on our behalf (such as our billing or scheduling systems) under a written agreement requiring them to protect your information

Uses That Require Your Written Authorization

Other than as described above, we will not use or share your health information without your written authorization, including for marketing purposes or the sale of your health information. You may revoke a prior authorization in writing at any time, except to the extent we have already acted on it.

Your Rights

Right to inspect and copy your health record
Right to request amendment of your health record if you believe it is incomplete or incorrect
Right to an accounting of disclosures we have made of your health information
Right to request restrictions on certain uses and disclosures
Right to request confidential communications — for example, asking us to contact you only at a certain phone number
Right to a paper copy of this Notice, even if you agreed to receive it electronically
Right to file a complaint if you believe your privacy rights have been violated, without fear of retaliation

Our Responsibilities

We are required to maintain the privacy and security of your protected health information, notify you if a breach occurs, follow the duties and privacy practices described in this Notice, and not use or share your information other than as described here unless you tell us otherwise in writing.

Changes to This Notice

We reserve the right to change this Notice and to make the revised Notice effective for health information we already have as well as information we receive in the future. An updated Notice will be available at our office and posted here.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
